NISS and SNS in Portugal

The two numbers you request after arrival: social security (NISS) and the health service (SNS).

The short answer

Your NISS is your Portuguese social security number, requested from Segurança Social once you are resident. Your SNS number registers you with the national health service through your local health centre. Both come after arrival, not before.

NISS — social security

The NISS identifies you in the Portuguese social security system. Employees usually have it requested by their employer; self-employed people and most new residents request it themselves, with their residence permit, NIF and identification. You will need it to work legally, to contribute, and for family and parental benefits.

SNS — healthcare

Portugal has a national health service. Registration happens at the centro de saúde for your address: take your residence permit, NIF, NISS and proof of address, and you will be registered and assigned a family doctor where capacity allows. Waiting lists for a permanent family doctor are real in some areas, which is one reason many new residents keep private insurance for the first year.

Your visa file required travel and health insurance. Most people keep a private policy after arrival too — not because the SNS is unusable, but because it shortens waits for consultations and diagnostics. Budget for it as a monthly cost rather than assuming it disappears once you are registered.

Residence permit appointment.
NISS request.
Health centre registration and SNS number.
Address update with the tax authority.
Utilities and lease in your own name.

Doing them in this order avoids the loop where each office asks for the document the next one issues.

Questions people actually ask

Do I need a NISS if I do not work?: You may still need it for family benefits and various formalities, and requesting it early avoids delays later.
Is healthcare free?: The SNS is largely publicly funded with modest user charges for some services. It is not a private-standard service and waits vary by region.
